2011 Program
Notification of results
The 2011 assessments have now been finalised and candidates will receive notification of results in the post after 4 October.
Candidates who have not passed are reminded of the following
- An application for Reassessment can only be submitted if there has been a failure in just one assessment component.
- An application for Appeal can only be submitted on the basis of alleged procedural error as experienced by an individual candidate during the assessment process.

Instructions for use
Please follow the instruction on the form to submit a completed application to the Specialist Accreditation department electronically or by post.
- Users of Adobe® Acrobat Reader 8 or 9 should be able to save information entered into the form.
- Answers can be reopened and modified as often as required using File/Save As, giving each iteration of the form a unique name.
Ensure an electronic or hard copy of the completed application is kept as a reference.
